Instructions for my next-of-kin and executors on my death - Information sheet 18

This information sheet is a form for you to complete and pass to the person who will deal with your affairs after your death.  By completing the form you will make the job easier and less stressful for them and also help to ensure that things are carried out the way you wanted them to be.  The form includes sections on:

  • Personal information;
  • Who to contact after your death;
  • Where to find your will;
  • Funeral arrangements;
  • Where to find important papers and items.
