Hospital discharge arrangements - Factsheet 37

This factsheet looks at how your discharge from hospital following NHS treatment should be managed. It looks at the care and support decisions you may be asked to make at this time. This factsheet looks at:

  • Identifying the help and support you will need on leaving hospital
  • Services that may promote your recovery 
  • NHS and social services to support you

  • Monitoring your needs and adequacy of your care package
  • Options if you are unhappy with the care and support you receive
